This training is a component of the prerequisites of the CPESN USA Heart Failure Management Enhanced Service Set Standard. It is designed to help pharmacies improve their ability to care for patients with HF by highlighting best practices, suggesting implementation ideas, identifying roles for each member of the pharmacy team, and describing successful methods to discuss HF management with patients.
